Introduction to the Survey

Dear participant,

Thank you for your interest in our scientific study on video-based counseling services for people with headache disorders.

Survey Structure

The questionnaire includes the following sections:

1. Questions about yourself and your headache experiences
2. A short video (approx. 2 minutes)
3. Questions about your impressions of the person in the video
4. Final questions and the option to provide your email address to receive the study results (optional)

Important note: Please ensure that your device's volume is turned on and set to a comfortable level, as a video with sound will be played.

Data Protection and Consent

As part of this study, your data will be stored and processed on a server within the European Union. The collection and processing is pseudonymized and complies with all relevant data protection regulations (GDPR). Your data will not be shared with third parties.

For technical purposes, we collect the following information about your device:

• Device provider (Android, Apple, etc.)
• Form factor (computer, tablet, mobile phone, etc.)
• Screen size
• Questionnaire form width

Information According to Scientific Standards

We hereby inform you about the following points relevant to scientific studies:

• Voluntary participation: Your participation is completely voluntary. You can withdraw from the survey at any time without giving reasons.
• Risks and benefits: There are no known risks associated with participation. Your contribution helps improve online counseling services.
• Confidentiality: All information is treated strictly confidentially and evaluated only in anonymized form.
• Results: The study results will be published in scientific publications without allowing conclusions about individual participants.
• Duration: Completing the questionnaire takes approximately 10-15 minutes.
